WAIVER OF LIABILITY, MEDICAL AUTHORIZATION, MEDIA CONSENT, AND CODE OF CONDUCT
By enrolling my child in CFC Academy, Cebu Football Club, Dynamic Herb Sports Inc., and all related programs, activities, training sessions, camps, clinics, tournaments, and events (collectively referred to as the “Academy”), I acknowledge and agree to the following:
- Assumption of Risk
I understand that participation in football and sports activities involves inherent risks, including but not limited to injury, illness, disability, property damage, or other unforeseen incidents. I voluntarily allow my child to participate and assume all risks associated with such participation.
- Release of Liability
To the fullest extent permitted by law, I release, waive, and hold harmless the Academy, its directors, officers, employees, coaches, volunteers, sponsors, partners, and affiliated persons from any claims, demands, liabilities, damages, losses, costs, or expenses arising from or related to my child’s participation in Academy activities, except in cases of gross negligence or willful misconduct.
- Medical Authorization
In the event of injury or medical emergency, I authorize the Academy to obtain emergency medical treatment for my child if deemed necessary. I understand that all medical and related expenses shall be my responsibility.
- Health Declaration
I confirm that my child is physically fit to participate in Academy activities and that I will inform the Academy of any medical conditions, allergies, injuries, or other health concerns that may affect participation.
- Personal Property
I understand that the Academy is not responsible for the loss, theft, or damage of personal belongings brought to any Academy activity.
- Media Consent
I grant the Academy the unrestricted right to photograph, film, record, and use my child’s image, likeness, voice, and participation for marketing, promotional, educational, commercial, social media, website, print, and other Academy-related purposes without compensation or further approval.
- Code of Conduct
I understand that players and parents are expected to conduct themselves respectfully and professionally at all times. Players must follow the instructions of coaches and staff, demonstrate good sportsmanship, respect teammates, opponents, referees, facilities, and equipment, and avoid any behavior that is unsafe, disruptive, abusive, or detrimental to the Academy environment. Parents and guardians are expected to support a positive atmosphere, refrain from interfering with coaching decisions, and treat all participants, officials, staff, and other families with respect. The Academy reserves the right to suspend, remove, or deny participation to any player or parent whose conduct is deemed inappropriate or contrary to the values, rules, and policies of the Academy. No refunds shall be provided in cases involving disciplinary removal.
- Academy Policies
I agree to abide by all Academy rules, policies, and procedures, as may be amended from time to time.
By signing below, I confirm that I am the parent or legal guardian of the participant, have read and understood this agreement, and voluntarily accept its terms on behalf of myself and my child.
Enrollment, payment, attendance, or continued participation in any Academy activity constitutes acceptance of this waiver and all Academy policies.
